Tourist Medical visas are not being issued at this time...and when they are its only for urgent surgical need with referral from your doctor with medical records and approval by Thai doctor and hospital acceptance of you as a patient.

Its for a set time period following which you are to leave Thailand.

Suggest you search for another visa correct for your planned stay in Thailand.
