I am planning to go to Bangkok for 2 weeks and afterwards to Phuket.
Does anyone know a place in Bangkok (Muay Thai) where I can request for the DTV visa only for a few classes or 1 month so I can go on to another island.
And also the question is do I need to apply for Bangkok or could I also apply for Phuket and can first enter in Bangkok ?

First of all, you need to apply from outside Thailand; you cannot be in the country during the application process. Also, forget about obtaining a 5-year DTV with just a 1-month subscription; you’ll need at least 6 months.
You are approved on the basis of the credentials of the first school you buy classes from. 6-months is the minimum program. That program will consist of a certain number of classes which you will have 6 months to take. So, you can certainly train in BKK for a month, go to Phuket for a bit, but unless you can find a school with multiple locations where you can use the classes, you will have to return to BKK to continue your 6-month program.
You won’t get accepted for a W week or 1 month course. You need to book a 6 month course and it’s up to you if you want to take the loss on the training or not
