If the form is required is immigration-office-specific. Plain A4 size paper is good for Bangkok for example.
Better to check your local immigration office or post to the local group around you..

Thanks a lot. Last time at Chiang Mai Immigration I had drawn the map on plain paper as I didn't realise they have a form for it. So in a rush I copied it over to the forms (two copies) and put it with my documents, forgetting to get an empty copy for next time.
If a form is required, it can be picked up at the immigration office. What the immigration office is looking for is the name of the cross streets you live on, and any landmarks (i.e
****
). Do NOT make this more complicated.
