TLDR : Answer Summary
The "hand drawn map" form for the NON-O Thai spouse extension is not available for download online. Instead, it can be obtained at the local immigration office, where you can also draw a map on plain paper if necessary. Each immigration office may have specific requirements—Chiang Mai, for example, provides a specific form with a designated box for the map. It's advisable to contact your local immigration office to confirm what's required.

IMPORTANT REMINDER: This information about the special from for the map is ONLY REQUIRED for Chiang Mai Immigration. If you're using a different immigration office, ASK your local immigration office. I do not see the point in keeping commenting open, if anyone disagrees, please send a moderator a message. Thank you.

It looks like this for Chiang Mai for anyone wondering. Located in the tray section with all the other forms

Thanks, there's a form with a box to draw within for Chiang Mai so I'll go and pick it up if it's not available online

If the form is required is immigration-office-specific. Plain A4 size paper is good for Bangkok for example.

Better to check your local immigration office or post to the local group around you..

Thanks a lot. Last time at Chiang Mai Immigration I had drawn the map on plain paper as I didn't realise they have a form for it. So in a rush I copied it over to the forms (two copies) and put it with my documents, forgetting to get an empty copy for next time.

If a form is required, it can be picked up at the immigration office. What the immigration office is looking for is the name of the cross streets you live on, and any landmarks (i.e

). Do NOT make this more complicated.

Tod Daniels has his map to his house in Bangkok posted somewhere in the group.
