Try online, i did and was ok! If rejected go to immigration office.

This is called TM-47 , however for TM - 47 you need first TM-30 . Better to go prepared with requeired documents

First floor on your right when you walk in, they have a seperate section for DTV holders

is it definitely in person first time?

first time for the 90day must be done in person, if you try online they will just reject you and ask you to go in person.
