I just went this morning (b/c I couldn't do online) and I did bring a filled out TM-47. I'm not sure if it was needed or not but they took it and didn't say anything so maybe it is. I went to the drive-through at the airport location and it was pretty quick.

passport, previous 90 day report slip, go to the drive thru at the main office by the airport. I don't even think you need the TM-47 form there anymore
