TLDR : Answer Summary
When filling out a passport application, the 'place of issue' field can be filled as 'Wojewoda' followed by the specific voivodeship, such as 'Wojewoda Mazowiecki.' It typically refers to the region where the passport was issued. If in doubt, contacting the Polish embassy for clarification is advisable.

You write (based on above) Wojewoda Mazowiecki. You can also call the embassy to double check - they answer all the question on the phone. But I am 100% that this is correct.

Usually, it would indicate the name of the province where your passport was issued (or Polish Embassy/Consulate)....
