Things I included:

*A cover letter stating my remote work, reason for wanting visa, and not only the documents I was including, but other documents I could provide if asked (such as tax documents, full portfolio, my employee contract, etc.) 

*Screenshot of bank account with current balance of needed funds and my name 

*Other recent paystubs showing continual income 

*A Remote Work Contract letter that I drafted and had my employer sign, stating my release to work remote and my role & responsibilities working remote. 

Hope this helps! Going to apply for my husband and daughter now.

yes all the normal requirements of proof of address, passport, photo, etc.

yes that’s correct. In the cover letter I provided, I listed some extra documents I could provide, but they didn’t ask me for anything extra

Be sure to PRINT OUT SEVERAL COPIES of the Evisa email you received to show Thai Immigration Officers upon arrival into Thailand during the 5 years that the DTV visa is valid.
