Here is the Form for the « Middle East War Flights Cancellations » Extension of Stay for 30 Days at your local Thai Immigration Office 

As a few Foreign Embassies/Consulates in Bangkok have already posted on their respective Facebook Pages that they will not issue Embassy Letters and the Form ( « Affidavit » ) is to be used in lieu of the Embassy Letter/Certification 

So far, the Dutch Embassy in Bangkok and the German Embassy in Bangkok have addressed the "Embassy Letter/Certification" for the "Middle East War Flights Cancellations" Extension

Doesn't say much for those embassies care for their citizens if they would prefer to leave them to the vagaries of thai immigration because they can't be bothered writing a letter
