O visa has no medical insurance requirements. OA does ..

If applied for in the UK (like all other visas to enter Thailand) insurance is required to cover its 90 day duration. But if applied for within Thailand and when extended thereafter no insurance is required.

You'll have to elaborate. There's about 4 or 5 different non-immigrant O visas. Which specific one are you referring to?
