if you have all the documents other wise an extra charge of 200 Thb could be asked to provide the documents.

At the airport they will do the form and photo for you. For 200 baht.

1 passport picture, copy id page, copy visa page, copy Admitted until page, application form as far as i know, have to go make a search on this page to get the complete list
