TLDR : Answer Summary
To complete your first 90-day report in Chiang Mai with a Non-O retirement multi-entry visa, you will need the TM30 receipt and a filled-out TM47 form. Generally, you do not need to provide copies of these documents.

do you have a year-long, multi-entry Non-Immigrant Type O-A visa <- the one that required a police background check, a medical certificate and that you got from the thai consulates in Chicago, Los Angeles, New York or Washington DC?

The kind that gets you stamped in for a whole year when you entered the country?

If it is multi entry you cant 90 day report you got to leave country i think๐Ÿ˜Ž

IF it was a year-long, multi-entry Non-Immigrant Type O visa you would be correct

BUT

Seeing as there are no consulates in the USA that will sell that visa type for that reason, it has to be a year long, multi-entry Non-Immigrant Type O-A visa <- note the A after the O.

That visa gets you stamped in for a whole year.

BUT just to be sure, let's ask the O/P

TM30 receipt, filled out TM47 (90 day report form) and that should be it.

I don' think you need copies of anything up there.
