I have an appointment booked in Laos next month. Trying to prepare exact documents needed. Applying with a 3 month Muay Thai course from a certified school. Where do I get the visa application form from to apply specifically in Laos?

FYI - finally got a response from the Thai Embassy in Vientiane. Confirmed that they required 3 months of bank statement and each month is required to be over 500K

Well, I am in Laos, perhaps there is something wrong. However, the error message comes from the tinyurl web server and is not a general internet error.
