Covid extension for full 60 days from Pai, MHS office. USA passport. Needed to provide a print-out of email from US embassy stating that they're not providing letters (same email they've sent as an automatic response for months). 1900 baht (there had been a lot of talk about them charging 3000 baht). I did get an initial UC stamp and lots of questions about my exit strategy.

nothing abnormal, one copy of passport first page and one copy of each extension. The covid extension forms, overstay forms etc can all be done in office.
