What documents and forms are needed to get a 30-day tourist visa stamp in Thailand?

Got my fresh 30 day TR visa stamp at Chaeng Wattana today (8/5/21). Process was super quick. Showed up at 14:30 and got out at 14:50. I was que number 100.
