TLDR : Answer Summary
When filling out your online 90-day report for your DTV visa, you should enter the date your current stamp expires, not the date you plan to leave Thailand.

It does matter if you leave the country. Every time you enter Thailand, the first report must be in person.

If it’s your first 90 day report, the online application will be rejected. You must go in person.

Why would you fill for a second 90 days then? Have you extended in Thailand? If you have left the country and back, you'll need to do it in person again.

so. You do the first 90 days in person. Then. The next 90 days. You have to leave. Per DTV. ?
