What do I need to apply for a multiple re-entry permit for an OA on its "second" year? Are forms available online? Does the visa holder have to apply in person at Chang Wattana?

Holder of the visa is in a wheelchair so tends to get whizzed straight to the gate at airports so a visit to CW is probably going to be necessary. Question is whether he needs to attend in person.
