You should ask for experiences with applying at Penang by evisa since anyone's experience before then isn't really relevant

I have personally done it in September 2024 at Thai Consulate General, Penang as soft power medical treatment. There is not much issue.

I submitted Hospital Appointment letter, Visa request letter from hospital.
