How should I fill out my DTV application if my passport has only my full name under 'given name'?

Hey! 

Sorry this might be a stupid question but I’ve been kinda confused so figured I’d just ask. 

I’m planning on applying for the DTV via the workcation/freelancer route. On my passport, we have the “given name” and the “surname” category. My entire name (first, middle and last) is written under “given name” on my passport and the surname section is left blank. 

I’m just wondering if I should leave the “last name” section empty in my application since my surname field is left empty on my passport and if I should fit my entire name between my first and middle name sections? 

PS - I mailed the embassy a week back but I haven’t really recevived a response, so I’m not sure if I’m gonna get a response from them.

You haven't said what nation your passport is from and where exactly the password was issued. And are those three names in the >>>>MRZ>> zone, how are they separated? This would be your first clue on how to supply the name. Without more information it's pretty hard to give you some advice, other than wait to hear back from the embassy you're applying through.

Good question. I think his passport is faulty. When I renewed my British passport in Bangkok they made me check each field was correct and sign to that effect before cancelling my old passport.
