How does the 30-day extension work in Thailand, and what should I enter in the TM 7 form for my arrival?

When applying for 30 day extension do you get 30 days added from your last stamp or 30 days from when you apply

TM 7 on the arrived by line do you put your border bounce date and place or original when you flew in?
