How do I upload flight details and vaccination certificates on the Thailand Pass?

Hi there.

Just a quick couple of queries. 

Regarding the Thailand Pass..

Firstly, when uploading flight details, how do you present the information into the relevant box? (I know it’s jpeg/screenshot, no PDF).

Do I need to put both outward and inward flight details? Do I have to crop the information to fit it into the box?

Secondly, I received my Vaccination certificate and there’s no QR codes on it, so do I upload from my NHS App (this has QR Codes) onto the Thailand Pass application?
