How can I obtain an OA visa if my Medical Certificate is not compliant with Thai Embassy requirements?

Help! I cant get my OA visa because both UpDoc and Medical Cert are using non compliant forms for the Medical Certificate. The correct form is on the Thai Embassy website but these companies are not using that form. Can anyone help me as I sm at my wits end dealing with these companies?

Where are you I’ve just done mine online in uk and it was easy just 24hr turnaround

My GP would not sign it or run the tests nor would a private local hospital I approached . But it was easy to do online .

they will not accept photos except for the passport

take a photo on your iPhone or iPad and then select print ( even though you are not going to print it). It will turn it into a pdf which you can then save and send.

For the Medical Certificate in the past I had the doctor sign it and put the center's stamp on it. Then I scanned it and uploaded into the Evisa website.
