Just received DTV SOFT POWER in Manila.

Took 3 working days. Staff very helpful and polite. Cost: 18.000 PHP 

In comment I added video in Polish language how to get it, my English friends ask me I will happy to answer all questions.

Yes i give them copy of my accommodation contract. Embassy send all requirements thru email

Roland from what my friends says immigration in phuket when you go for extension they require all papers. So general advice is to do border tun.

1. 3 month bank statement with 500,000+ THB balance (I used USD currency)

2. medical records + document prove follow up visit

3. accommodation contract (I have yearly contract)

4. visa application form

5. 2 photos 3,5x4,5cm

6. copy of my passport image page

7. copy of my filipino visa

8. copy of my filipino visa extension
