I've tried over 100 passports of people and have about an 85-90% success rate.
The criteria to get it to work are
turn OFF any pop up blocker programs
you need to have done at least ONE 90 day report in person or by mail
you need to be in an area where the immigration office is participating in the system <- it's country wide but some offices opted out. :/
you need to apply online from 15 days before down until 7 days before
IF you have done all those you should be able to get the system to work.
**ONLY fill out the fields that have a red asterisks on the first page.
you can go when ever you want to they're open from 8:30 - 4:30, closed from 12 - 1 for lunch and stop giving out queue tickets at 3:30 in the afternoon>

, seeing as it's your first time you can't use the online program AND you're too late to mail it in too (has to be postmarked 14 days before your due date).
You DO go to Chaengwattana and can't use Cham Churi.
BUT
you don't need to take your dependent with you and can do a 90 day report for them without a problem with just their passport and a filled out TM.47 form
I'm guessing I cannot use this system as this is my first time doing a 90 day checkin (deadline April 6, 2019, 6 days from now). Do you have the link or instructions for the mail in form? Do I have enough time to do it? If I do go in person, do I have to go to Chaengwatthana (where I usually go for immigration) or could I do it for Chamchurri Sq. I ask because I also have a dependent, and I would rather not have to pull her out of school to do the check in.
IF you can't get it to go thru filling only the red asterisks stuff AND using the calendar drop down to pick the dates (not filling it in) I dunno. I got no answer for you then. Sorry. :(

Always return by air. My last return was may last year.
and did you enter the country AFTER 2012 by air OR after 2014 by land? Those seem to be the cut off dates of having your TM.6 departure card in the system. before those dates you can't use the online program.
Sorry, still not loading page two. Here's what happens on Safari, Chrome and Firefox... I get a pop up window, then the page loading times out. If I close the popup, it returns to page one.
I have tried several times to do this online, following your instructions. It doesn't take me to the second page, just hangs there saying it's loading the next page, but never does. I've tried 3 different browsers. I use a mac, is that an issue?

my province is Mae Hong Son, is there a list of participating provinces?

You can use this, after you made your first report in person, asked the Immigration Officer if their office participate in the on-line reporting. Than you can fill in questions between 15 and 7 days before your due date.
