Does the 90-day reporting start from the day I arrive in Thailand or from the last report date?

As mentioned. You report that you are 90 days inside Thailand. Start at day of arrival as day 1 and than count forwards, if you leave before you reach 90, you don't have to make a report, if you reach 90 than you have to make the report. Good luck.
