I'm planning to get a re-entry permit at Chiang Mai airport. Are the application forms available at that desk? I'm not able to print myself.

TLDR : Answer Summary
The inquiry is about obtaining a re-entry permit at Chiang Mai airport and whether the necessary application forms are available there. Responses confirm that the re-entry permit counter at the airport does have the TM8 form available for those who need it. Additionally, some suggest paying a fee to have assistance with the process.
Will depend on if it is a direct international flight from Chiang Mai as to whether you can use immigration at the airport. Everything can be done there including the forms and photos.
Examples:
You fly from Chiang Mai to Bangkok
You fly from Bangkok to another country
You must get your re-entry permit at the Bangkok airport
or
You are flying from Chiang Mai direct to another country
You can get your re-entry permit from the Chiang Mai airport
, it depends on your type of tickets, not the itinerary
Even if you go through BKK airport, if your tickets are connecting type, you get re-entry permit and are stamped out from the country at CNX.
If your tickets are not connected, you need to get re-entry permit and are stamped out from Thailand at BKK.

Thanks Mike. Yup, flying out of the country, so will take care of business at the airport.

Just pay the extra 200 baht and let them do it

I have never used that re-entry permit counter
BUT
I'd bet dollars to durian that they have the TM8 application for re-entry forms there for people who show up to buy one before flying out internationally
Hopefully someone who has used that re-entry counter will weigh in with the answer
