I'm planning to get a re-entry permit at Chiang Mai airport. Are the application forms available at that desk? I'm not able to print myself.

You can get your re-entry permit from the Chiang Mai airport

Even if you go through BKK airport, if your tickets are connecting type, you get re-entry permit and are stamped out from the country at CNX.
If your tickets are not connected, you need to get re-entry permit and are stamped out from Thailand at BKK.

I have never used that re-entry permit counter
BUT
I'd bet dollars to durian that they have the TM8 application for re-entry forms there for people who show up to buy one before flying out internationally
Hopefully someone who has used that re-entry counter will weigh in with the answer
