Where do we do the 180 day/6 month extension? Can any immigration office handle this? Or do we do it online?

Thanks for any information!

TLDR : Answer Summary
To apply for the 180-day extension, you must visit the immigration office that serves your area in person. It's important to prepare your documents and maintain a minimum balance of 500,000 baht in your bank account for at least one month prior to your application. Alternatively, some people choose to leave the country and re-enter to obtain another 180 days.

You have to do it in person at the immigration office that serves the area you live in. You need to update all your documents and have 500k baht in a bank account for a full month before you apply. Most people just bounce out of the country and back again to get another 180 days, even immigration offices recommend this.
