The application is free, and you don't pay until you are approved and go to pick up your visa, so the best way to do it is just start the application. Then if you have any questions you can contact the BOI directly and they are usually very responsive to questions.
You look like you may be 50 years or older. If you are 50+ the you could apply for a non-O visa + a 12 month extension of stay. This would give you 15 months in TL and you don't need to leave the country every 180 days. Then you just renew the extension of stay every 12 months. Do you need the DTV?
