I'm guessing you can get the medical certificate at a hospital, and for the insurance requirements, I'm guessing contact an insurance agent.

This is a requirement for all countries. I'm attaching the infographic about it today as well.

Because I can't verify if a certain insurance company sells the policy or not, and because each insurance policy is individual specific, I'm going to close commenting. The other mods can reopen it if they see fit.
