What is the process for obtaining prescription medications as a UK expat in Thailand?

So I'm from the UK, and have a question for all the expat OAP's who are on 20 tablets a day like myself? What's the process for prescription meds? Do I need to visit a hospital with my medical history and get it OK'd by a doc first, or can I just stagger out of Soi Cowboy into the nearest pharmacy, no questions asked? Tia
