For those applying under medical, do note that they will ask for your doctor's medical license number and ID and a history of your medical appointments. I thought I'd mention this so you can submit with your application and don't waste precious time to get it and resubmit.

Anonymous participant 914 he and I haven't found on any website that states the requirement but he did have his doctor write something to the effect that his condition requires ongoing care and his next appointment date. I also have read that it'd be prudent for your doctor to say that you'll need treatment for at least the next 6 months or else you might end up with an METV or flat out rejected. My husband's doctor did NOT write this portion but if you want to be extra sure, I'd try to get your doctor to write this.
