What documents do I need to apply for a Soft Power Medical visa in Thailand?

Hey everyone, so need a bit of advice here, I'm about to apply for soft power medical, I went to Chiang Mai hospital and started a treatment with them for my skin condition, also did some blood tests , etc. I have so far from them receipts for payments made to consultations, tests and medication, statement document indicating my name and passport number along with condition , treatment to be taken , duration of treatment and required follow ups, and appointment slip paper indicating that I need check ups every month for 12 months. Do I need anything else from them, also the documents are signed but not stamped. What do you think ?

I was recently approved under soft power medical in Chiang Mai at Bangkok Hospital.

Documents I submitted:

- medical certificate/receipt from day of medical examination

-detailed doctors report that showed past history of treatment, current diagnosis, and future treatment plan

-proof of upcoming appointments with receipt showing future treatment is prepaid already

My future appointments showed six weeks of upcoming treatment.

My documents were stamped by the hospital (I didn’t ask for that. Just seems to be standard at Bangkok Hospital.)
