What documents do I need for an Extension based on Tourism at Chaengwattana?

I hope someone's awake.

I'm going to Chaengwattana this morning (in an hour or 2) to queue for the Extension based on Tourism. What do I need to bring apart from my passport and the fee? Also, I lost my departure card but I have the police report.

Please advise. Thanks!

In Bangkok you get your extensions on visa exempt entries at the new immigration location on the 3rd floor at IT Square Laksi Plaza here
