I would like to get an acknowledgment from people who had experience with a DTV based on medical treatment.
Anyone knows what are the exact documents need to be submitted for a Medical DTV - my partner is pregnant and we are planning to give birth in Thailand.

Apart from the documents listed in the application and those listed by other commenters, the embassy also requested Thai affidavit from my clinic (applied under dental treatment for braces) as well as a copy of the front and back of my identity card. I’m a Singaporean and applied in Singapore.
Typically you need to provide proof of ongoing care, diagnosis, treatment plan, at a recognized medical facility with appointments set up for the next 6 months at least.
