i just did my third one and had massive problems. in the end it got solved and i got it but took like a total of about 14 hours over a period of days.
reason for it was apparently that i did mine previously on koh samui and than changed my location to nong khai/ isaan ?!?! and the officers even called surat thani and bangkok to confirm.
if you can do it.. always do it in the same location to avoid any problems.

ORIGINAL POSTERyes. same documents you had to submit your first time you have to submit again. do it at the same immigration office you did it your first time!
