For the LA Consulate for DTV under soft power, if one gets requested for a photo again because the first photo wasn't good enough but appears to have accepted the bank statement and soft power welcome letter - how long after submitting the second photo do they usually approve? and also how many times can/do they reject photo before totally rejecting application? they rejected photo and asked for another one in 1 day, but ever since uploading the 2nd photo, waiting nearly 5 days now under "processing/pending". . appreciate your feedback thanks

Who knows? It is what it is, document requests reset the processing time. I'd concentrate less on how long it's going to take, and more on providing a good digital passport style head and shoulders, no smile, no glasses, off white background, photo less than three megabytes.
Generally any request for documents will move your application to the bottom of the pile. No one can really say how long it’ll take but keep in mind that applications are only processed on business days.
If they reject the photo again then it’s just a repeat of the process. My personal advise would be to just get a passport style photo taken at a business and get a digital copy of it as that is what they’re looking for.
