Does anyone have experience with filling out a ED Visa Application Form? I would appreciate some advice on exactly how I should fill out the segments pictured.

My history in Thailand:

June 25 2019 - arrive by air - 30 day exemption

July 24 leave by air.

July 26 arrive by air on a SETV from HCMC (extended by 30 days for 90 days total)

Border run to Myanmar and back 16 October 2019

Depart Thailand by Air 26 October
