Choosing a phone number in Thailand can be confusing due to superstitions surrounding numbers. It's possible to select a number based on personal preference or avoid certain sequences. There are physical stores that provide spreadsheets of available numbers, and clicking 'search' may automatically assign you a number.

Is it even required to fill out? Can't you just click search and it will assign you a number?
Thais are very superstitious about numbers and that's what the 2 boxes are for, a sequence of numbers you want within your phone number or a sequence of numbers you want to avoid in your phone number.
The physical stores have a big spreadsheet of numbers you can pick from. I found it very awkward. I just wanted a phone number 😂
it's usually a personal thing. Some big event in their life or some number they think they received from a special divination or something.
If you've ever seen Thais lighting little blocks and then putting them in sand near a shrine, when they finish burning there is a number on it. Then they will go play the lottery with this number.
There's also a cup full of sticks and they shake it and the first ones that fall out they use those numbers etc. They're very superstitious.
Also "lucky number" lottery tickets fetch higher prices and unlucky numbers may be discounted. I think repeating numbers are thought of as lucky but I honestly don't know much. I'm not superstitious so I've never looked into it.
