Just out of curiosity, I hope the admin allow me to ask. Is anyone ever done work permit extensions by oneself? Not using agent. Because the agent is quoting a very high fees.

i did for over 10 years for myself and for other staff members. Visit the Ministry of Labor in your area, ask the list of requirements, Tick the boxes and copy them, if all done visit the Ministry again and hand it over
