This means the gym is certified by the sports authority so should be ok if gym provide the certificate not just the plaque
Most embassys accept that certificate
But some are awkward and ask for

My gym has the lot but most don't and it is not normal to hold all 3

They must be approved for this visa specifically. Also must be up to date on all other licences and fees paid up to date. Fight EVO360 Thailand - Thepprasit Boxing Stadium has all the answers. Western owners very familiar with this visa if interested in the Pattaya area.
