TLDR : Answer Summary
A gym certified by the Ministry of Tourism and Sports can potentially issue a letter for a DTV visa, provided it is approved for this specific visa. While most embassies accept this certification, some may insist on additional credentials from the Ministry of Education or the Board of Boxing Sport. It is not uncommon for gyms to lack this full set of certifications, but the sports authority certificate is generally the most critical for the visa process.

It is possible to get without the MoE cert

This means the gym is certified by the sports authority so should be ok if gym provide the certificate not just the plaque

Most embassys accept that certificate

But some are awkward and ask for

Board of boxing sport or ministry of education

My gym has the lot but most don't and it is not normal to hold all 3

I think the sports authority certificate is the main one requested though

They must be approved for this visa specifically. Also must be up to date on all other licences and fees paid up to date. Fight EVO360 Thailand - Thepprasit Boxing Stadium has all the answers. Western owners very familiar with this visa if interested in the Pattaya area.
