What Should I Put in the 'Type of Visa' Field on My TM7 Application?

Hello again!

I’m in the process of filling my TM7 and I’m in sure of what to put in the field “type of visa”, as I originally came on a OA-M visa, but I am currently on a one year extension with multi entry permit. Do I still reference the original visa and the dates on that?
