Notes: DTV application cost showed up as $343 on my card.
Visa on arrival for Indonesia was $32.
3 and a half hour flight from Bangkok, approximately.

-Letter of acknowledgement from the state of Texas about my company registration.
-Receipt from when I made the payment for the company registration, to the state of Texas.
-My credit card statement from that month showing that payment.

I'm in Jakarta as well, submitted Sunday night as soon as I arrived and status is still "Pending Document Check (1)". Hopefully will see some movement today. Good luck Brother!
At this point they can still request additional docs or interview but from what I've read people are usually approved in Jakarta within 2-3 days of "Pending Approval" status.
