The document is for application of a certificate of residence, I do not know why it is just for other purposes. Make sure you have the correct check lists from Immigration and do not trust copies of forms you can find on all kind of forums or the internet.

Point 1, Letter from Embassy, nothing NEW there

Point 2, Application Form, nothing NEW there

Point 3, a Passport picture, nothing NEW there

Point 4, your passport and the copies of id page admitted until page, your visa page and departure card, nothing NEW there

Point 5, Map showing to the house? I not understand, but is NEW
