Setting up a bank account with Visa Exempt? I've heard of people doing it with a Tourist Visa, but what about on a 30 day visa exempt stamp? I have a 6 month rental lease & SiNet contract in my name if that helps. So, which banks/locations in Chiang Mai am I able to set up with?

Ideally, I want to use Bangkok Bank because my US employer can ACH me directly but I'm open to other options as well.

TLDR : Answer Summary
Yes, it is possible to open a bank account in Thailand with a 30-day visa exempt stamp. There is no specific prohibition in Thai banking law against this, but the final decision rests with the manager of the branch you visit. It is advisable to visit different branches if you encounter any resistance. You will need to provide proof of residence, which can include a notarized letter from your consulate or documentation from the immigration office. Bangkok Bank is a preferred option for some due to its ability to receive ACH payments directly.

There is nothing in thai banking law that prohibits opening accounts on a 30 day visa exempt entry. The decision on whether a branch will or won't allow it is done by that branches manager. If you get one that says no, walk to the next branch and try.

You will need proof of residence (either from the immigration office or from a notarized letter from your consulate) but you definitely can open an account on a 30 day visa exempt entry

Just bear in mind that a residence cert is likely to be quite a bit cheaper than an letter from your embassy. But if you are in Bangkok you may not have a choice.

what is your nationality? Aussies fill out a stat dec, Americans fill out a notary form.

It's either a "letter of reference" or just a copy of your passport with a stamp and signature. In both cases it basically just confirms you are who you say you are :)

They'll know what you're after.

How do you get the notarized letter from your consulate? (And what kind of letter?)

The letter from the embassy makes the process easy.
