Thank you for all the helpful advice regarding the DTV. I have a confirmation question, I wish that the answer is yes ;-)

If a spouse is included in a DTV visa, can she/he travel alone without the 'primary visa holder'?

The spouse DTV is simply for qualification. Once the visa is issued, it belongs to the person it was issued for and is separate from theirs.
