DTV Address Verification (US Citizen)

The Thai Consulate (e-visa) requested a utility bill or driver's license for address verification to process my DTV. My driver's license is from another state and all the utility bills are in my dad's name.

Does anyone have any experience using alternative documents for address verification?

I am from the states, have US passport. My wife is US perm resident, and has Nigerian passport. I'd like to get her a MEV for a 6 month trip in Oct. to Thailand.

I was looking at requirements, and how to get her a visa, and I found this online:

Visit the Royal Thai Embassy or Consulate closest to your residence in the US. (I guess they come out to PHX every month or 2 from LA, for 2 days)

Submit the required documents, including:

1. Completed visa application form <- which form is this?

1. Valid Nigerian passport with at least 6 months validity

3. One passport photo meeting specific requirements

4. Proof of sufficient funds: <- how much for 6 months?

Can it me in our joint bank account?

Bank statements showcasing your ability to support yourself during your stay in Thailand (amount varies depending on your planned duration and itinerary).

5. Proof of onward/return flight tickets <- really? I won't have any flight tickets yet.. 3 months in advance.

6. Proof of accommodation details <- Huh? We want to travel around, won't have this all arranged...

7/ Travel insurance policy <- OK

8. US Permanent Resident Card (copy)

9. Visa fee: Payable at the embassy/consulate in cash or money order <- anyone know what the fee would be?

Does the above look accurate? Any idea if the visa would be issued same day? How long does it take?

So as a US citizen with US passport, what would be the process for me, vs my wife?

EDIT : Immigration called me about 20 mins after leaving admitting their mistake. I returned and was given a 30 November stamp. However, this sheds light on the procedure for desired extensions from 30 November - 30 December, as I was indeed given the option, granted all required docs / embassy letter / another 1,900 baht would be required.

Reporting back from Samut Prakan Immigration: US citizen already previously applied for Covid extension prior to 26 September, returned to close the loop on 29 September, given 26 October expiry stamp.

Went back today 12 October to get the new “30 November” stamp (although I’d heard previous accounts from SP Immigration that they were now giving 30 December): told I have 2 choices:

1. Do nothing and leave 31 October or 2. Re-submit ALL previous documents required for previous extension, including Embassy letter, as well as 1,900 baht, and I will be stamped to 30 December.

I read in someone else’s comments on a previous post that they received a 30 December stamp, but I did not read that they needed to re-submit all docs / 1,900 baht again for a new extension - so perhaps they have switched up the requirements even since then.

Would appreciate if anyone else that’s visited SP Immigration recently can speak up or if admins can provide any insight. Thanks as always 🙏🏼
