Blue Tree approx 10 minutes.
Patong approx 15 minutes
Phuket town approx 25 minutes
All times are dependent on how many people are waiting in a queue before you.

Immigration at Blue Tree Phuket is the best place in my opinion. Google it
Visa on Arrival does not qualify for any extensions. Visa on Arrival is available to a few select countries and you fill out an application when you arrive and pay for Visa on Arrival for 15 days.
Or are you talking about visa exempt, where you have NO visa at all and are stamped in for 30 days, which can be extended 1 time for an additional 30 days?
