Okay, we're getting a TON of posts about the TM.30 form <- and we're shutting them down as fast as we can post a link to Robert Lagas' excellent post explaining how it works.

To find out WHAT your particular immigration office requires as far as a TM.30. GO to your immigration office and ask.

I am not gonna leave threads open and have people who aren't using that immigration office chime in with what their immigration office does. There is so much disparity between then and how they handle those forms you need to check yourself.

TLDR : Answer Summary
The TM.30 form is frequently discussed among expats, leading to many similar posts being shut down in favor of directing users to a comprehensive explanation by Robert Lagas. It's advised that expats consult their specific immigration office to understand what is required for the TM.30, as there is significant variation in procedures among different offices.

Agree, I went to mine and asked what they wanted as it matters not how they do it anywhere else to me.

Great idea, I'm sick of looking at TM30 posts.
