Is anyone aware of some rehab/addiction centers that are for Thai people (peasants) not ultra wealthy or farang?

Jack Kawakami Long it might technically be accurate and understandable as language but it can also be considered as an informal and derogatory term in English. Some people get quite offended by it's use (as seen in these comments). However, they lack the understanding to read your comment in it's context, which indicates you're trying to help someone out.

In practice, I have no idea how it works, but apparently you can get screening and assessment at local general hospitals (in conjunction with Local Government) and then there is a pathway to rehabilitation programs. You may have already tried this, but see last slide for how in theory works:

I know the local police gather up locals involved in drugs from time to time... placing them in 10-15 day rehab programmes
