I have to go to "engwattana Immigration Office" to get my new visa on Wednesday. The email I received says I need to get an ATK test. However it doesn't say if it needs to be certified or anything like that.
Can I just go to a pharmacy and take an atk test?
Or do I need to go to a hospital / clinic?

I donβt think so; Iβve been to Chaengwattana several times this year (last was April) and even though there were signs and even a booth for ATK testing I was not required to do a test. The only mandatory thing is a health declaration which you will fill in before entering at Gate 2 Building B. If you want to be absolutely certain then do an ATK test before going, but a note from a doc or pharmacist is not necessary.
