What are the requirements for obtaining a replacement driving license in Thailand after losing it?

Hi

Any one had to get a replacement driving licence x 2.

I lost my wallet Saturday.

My two licenses were 6 months old from rayong.

I am getting different answers about what forms are actually needed.

Rayong transport centre closed until Wednesday.

I have police report.

I know need 2 x passport

2 x extension

I have copies of both my lost licence

Some are saying on google need .medical cert and residents cert

Could be you need a police report. On the other hand it's registered in their system. There also is an app. Think still in Thai language but after registering you have some Icons. Ask Thai or google translate. The app is known and accepted by police.
